Information collected automatically

This website is a static page. It sets no cookies, runs no analytics or advertising trackers, and stores nothing in your browser. There is no account, no login, and no profile.

The site is hosted by Netlify, which, like all web hosts, records standard server request data such as IP address, browser type, referring page, and the date and time of the request. This data is generated by the act of loading any web page and is used for delivery, security, and troubleshooting.

Fonts loaded from Google

The typefaces used on this site are served by Google Fonts. When a page loads, your browser requests those font files from Google, and Google therefore receives your IP address and browser information. No other data is shared with Google through this mechanism.

The contact form

The "Contact the Doctor" button opens a form hosted by Google Forms. The form collects:

• Email address (required)
• Name (required)
• Phone number (required)
• A selection identifying the sender as a potential new patient, a company, an attorney, or other (required)

The form is an initial point of contact for new inquiries and for professional or administrative matters. It is not monitored continuously and is not a secure channel for medical information. No clinical information is requested, and none should be submitted.

Submissions are received in a paid Google Workspace account operated by this practice, under a Business Associate Addendum accepted with Google. Google Forms is included in the functionality covered by that addendum as part of Google Drive.

Information submitted through the form is used only to respond to the inquiry. It is not sold, rented, or shared for marketing purposes.

If clinical information is submitted despite the notice above, it will be handled through appropriate channels and removed from the form record.

Links to other sites

This site links to Universal Pain Management and to Google Forms. Once you follow a link away from rcpainmd.com, this policy no longer applies, and the privacy practices of that destination govern.

Your choices

Because the site sets no cookies and requires no account, there is nothing to opt out of in order to browse it. If you have submitted the contact form and would like your submission deleted, or you want to know what was retained, call the number below and ask.

Children

This site is directed to adults and is not intended for use by children. No information is knowingly collected from anyone under 18 through this website.
